Claimant Identification and Current Check Re-issuance Fees
For Individuals:
- Copy of government issued photo identification, e.g., Driver License, Passport, etc.
- $52 for check re-issue.
For Business Owners:
- Copy of government issued photo identification, e.g., Driver License, Passport, etc.
- Business Card
- $52 for check re-issue.
For Corporations:
- Copy of government issued photo identification of claimant, e.g;, Driver License, Passport, etc.
- A notarized affidavit completed by an officer and/or business owner allowing you to claim such monies as an employee of the entity.
- For change of address, a statement on the entity’s letter-head, indicating such change.
- $60 for check re-issue
For Third Parties:
- A notarized Special Power of Attorney for Agent to collect unclaimed property on behalf of claimant.
- $86 for check re-issue
Per California Government Code Section 50050 regarding Unclaimed Property, money which remains unclaimed in the treasury of local agency for three (3) years will become the property of the agency after a notice is published and no claim is received.
