Please call (951)736-2386 (951)-736-2386 with questions, or to make an appointment. The agent assisting you will confirm your appointment location, time, and any required materials you need. Due to an increase in passport application volume, acceptance agents may not immediately answer the phone. Please leave a message and a passport acceptance agent will return your call by the next business day.
Passport Renewals
PASSPORT RENEWALS: Most renewals can be done by mail or online by using the following link: Renew my Passport Online (state.gov). If you have a passport that was issued less than 15 years ago and you were at least 16 years old when it was issued, you may be able to submit a self-service renewal. Please visit the U.S. Department of State - Passports by Mail page to see if you qualify to use the service.
New Passport for Adult or Minor 16+
For New Passport for Adult or Minor age of 16 or over, all applicants must appear in person, and will need the following items:
1. Completed (but not signed) Passport Application Form (DS-11). [Download Form]The application must be completed in black ink. Please do not use corrective tape or fluid to correct errors on the form.

2. Proof Of United States Citizenship Or Nationality -
If you were born in the United States you must submit:
♦ Certified original birth certificate (birth certificates must contain the name of the parent(s)), or
♦ Previous U.S. passport.
NOTICE: California Abstract birth certificates, Hospital Birth Certificate, or birth certificates that do not list your parent's name(s) are not accepted
If you were born in a foreign country you must submit:
♦ Original certificate of naturalization or citizenship [may not also be
used as proof of identity], or
♦ Previous U.S. passport, or
♦ Proof of derivative citizenship.
3. Proof Of Identity -
♦ Current driver’s license, or
♦ Recently issued certificate of naturalization, or
♦ Recently issued government ID card.
4. Photographs - One 2" x 2" square photo, full faced with plain white or off-white background.
Passport photos are available at the Library for $15.00.
5. Photocopies of ID - Photocopies of the front & back of the ID mentioned in Step 3 must be submitted with a passport application. The copies must be single-sided and legible. Copies can be made at the Library for an additional $.15 per copy.
6. Passport Fee - The State Department portion of the Passport Fee must be paid by Check or Money Order Only. The Library Fee can be paid by check or money order, cash or credit card (Visa/MC/Discover).
For minors 16 & 17 - at least one parent or guardian must be present to sign and present ID.
New or Renewal Passport for Children under 16
For New Or Renewal Passport for a Child Under the age of 16, the child, and both parents, or the child's legal guardian(s) must appear and present the following:

2. Child's Proof Of United States Citizenship Or Nationality

The Child's Certified Birth Certificate showing both parents' names, or
Certified Court Order of Guardianship
4. Show Valid identification for BOTH parents
♦ Drivers Licenses
♦ U.S. Government Issued ID Cards

If both parents cannot appear in person - The non-appearing parent may complete and notarize form DS-3053 or submit a notarized letter authorizing the other parent to apply for the child's passport. All letters & DS-3053 forms must be filled out in black ink and must include a legible photocopy of the front and back of the non-applying parent's valid driver's license or government issued ID.
